A new extracorporeal piezoelectric lithotripter was tested for its gallstone disintegration capability. A total of 177 surgically removed gallbladder stones were submitted to piezoelectric shock-wave treatment. Prior to shock-wave application, the diameter, weight, and volume of all the stones and CT density and MR signal intensity of selected stones were determined.
Successful fragmentation, defined as all residual gallstone fragments being less than or equal to 4 mm in greatest dimension, was achieved in 14 of 20 animals overall, but in 10 of 10 animals in which focal point refocusing had been used.
All gallbladder stones were disintegrated into sludge or fragments with diameters of no more than 8 mm. In six of the nine patients the fragments disappeared completely within 1 to 25 weeks.
